Skin Graft Behind the Ear: What to Know

The skin behind the ear may be used as a wound site or, in some procedures, as a donor area. Its folds can make dressing placement and cleaning more demanding. Nearby hair, mask loops, hearing devices, spectacles, and pillow contact may rub the skin. The clinician considers the wound depth, available skin, blood supply, and how movement of the ear could affect the graft.

Early swelling, tenderness, crusting, or a difference in colour can occur. The graft may not match the surrounding skin exactly, and the scar can feel firm or itchy as it matures. Healing is assessed over follow-up visits rather than by one early view.

Keep the dressing dry and in place until the treating team gives different instructions. Avoid scratching, picking crusts, tight mask straps, headphones, hearing devices, and hair products that touch the wound unless cleared. Sleep in the position advised, and avoid bending or activity that causes repeated pulling around the ear. Clean only as directed; do not place cotton buds, oils, antibiotic creams, or antiseptics into the area without approval.

Bring any prescribed medicines and dressing supplies to follow-up. Tell the clinician if work, sport, or a device makes protection difficult so the plan can be adapted safely.

Seek prompt advice if the graft becomes loose, turns increasingly dusky, develops spreading redness or warmth, or produces cloudy or foul-smelling drainage. Fever, worsening pain, repeated bleeding, or swelling that affects the ear or hearing also needs assessment. Heavy bleeding, severe swelling, or feeling faint warrants urgent care. Avoid waiting for a routine appointment when symptoms are rapidly worsening.

Ask whether the graft is covering a wound or being taken from behind the ear, how to protect it from sleep and devices, when hair washing is allowed, and what changes should trigger a same-day call.
