What should you know about skin and nail changes of psoriasis?

Psoriasis is an inflammatory condition in which skin cells build up quickly. Plaques may be dry, red, raised, peeling, or covered with silvery scale, often on the elbows, knees, scalp, or lower back. Itch, cracking, and soreness can occur. Nail psoriasis may create tiny pits, roughness, yellow-brown discolouration, thickening, or lifting from the nail bed. A fungal nail infection can look similar, so examination and testing may be needed.

Moisturise thick plaques and cracked areas, avoid picking scale, and protect nails from repeated trauma or harsh chemicals. Keep a record of new areas, joint stiffness, and changes in nail texture. Treatment depends on extent and location and may include prescribed creams, light therapy, or medicines that calm inflammation. Do not assume every rash or nail change is psoriasis; a clinician can distinguish it from infection and eczema.

Arrange a dermatology review for new, spreading, painful, or bleeding plaques, persistent scalp involvement, or unexplained nail separation. Seek prompt care if many areas become intensely red and tender, you feel unwell, or joint swelling and morning stiffness accompany the skin changes. Early review can help protect nails and identify joint involvement.

Ask whether the skin and nail pattern supports psoriasis, whether a nail sample is useful, and which treatment suits the affected sites. Mention heel or finger pain, stiffness, and any impact on work or daily activities.
