Signs of miscarriage without bleeding

A change in pregnancy symptoms alone cannot confirm loss, because symptoms naturally vary and may ease for reasons unrelated to miscarriage.

Possible changes include reduced nausea or breast tenderness, pelvic pressure, cramps, fluid or tissue from the vagina, or no obvious change at all. These signs are not diagnostic. The pregnancy location, development and heartbeat may be checked with an examination, pregnancy testing and a transvaginal ultrasound.

A single assessment may not answer every question when the pregnancy is very early or dates are uncertain. Your clinician may recommend repeat testing or another scan before confirming the diagnosis.

Contact your maternity team, gynaecologist or urgent care service for advice if you are worried about a possible loss. Tell them how far along you may be, the date of your last period, any pain or discharge, and whether you have had previous pregnancy complications. Do not stop prescribed medicines without advice.

Seek urgent care for severe or one-sided abdominal pain, shoulder-tip pain, fainting, dizziness, fever, or heavy bleeding. These symptoms can signal a complication that needs rapid assessment, including ectopic pregnancy. Emergency review is appropriate even if you have not bled.

Ask what the examination and scan show, whether repeat testing is needed, and which symptoms require emergency help. If loss is confirmed, ask about waiting for the pregnancy tissue to pass, medical management or surgical management, and what follow-up will be offered.
