What these signs can mean

Breast softness, frequent feeding, cluster feeding and shorter feeds do not prove that milk is low. Babies can feed efficiently, and breasts often feel softer as feeding settles. The more useful clues are a baby’s growth, swallowing during feeds, urine output and overall alertness. Shallow attachment, tongue movement difficulties, illness, separation or infrequent milk removal can reduce transfer or production.

Offer feeds responsively and check that the baby’s mouth takes in a deep, comfortable latch. Listen for swallowing and keep the baby close during feeds. If advised, expressing after feeds or offering expressed milk may help protect supply while the cause is assessed. A Lactation Consultation can observe a feed, assess attachment and make a feeding plan. Avoid changing supplements or medicines without professional guidance.

When to seek help

Contact a clinician promptly if the baby is hard to wake, feeding poorly, has markedly fewer wet nappies, appears dehydrated, is repeatedly vomiting, or is not gaining weight. Seek urgent care if the baby is floppy, breathing abnormally or difficult to rouse. Get same-day feeding support when you are worried rather than waiting for the next routine visit.
