Signs of low milk supply

Short answer
Possible signs of low milk supply include poor weight gain, fewer wet nappies than expected, persistent unsettled feeding and breasts that do not seem to refill between feeds.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Best clues
- Growth, swallowing, urine output and alertness.
- Support
- A feed observation can identify latch or transfer problems.
What these signs can mean
Breast softness, frequent feeding, cluster feeding and shorter feeds do not prove that milk is low. Babies can feed efficiently, and breasts often feel softer as feeding settles. The more useful clues are a baby’s growth, swallowing during feeds, urine output and overall alertness. Shallow attachment, tongue movement difficulties, illness, separation or infrequent milk removal can reduce transfer or production.
Steps that can help
Offer feeds responsively and check that the baby’s mouth takes in a deep, comfortable latch. Listen for swallowing and keep the baby close during feeds. If advised, expressing after feeds or offering expressed milk may help protect supply while the cause is assessed. A Lactation Consultation can observe a feed, assess attachment and make a feeding plan. Avoid changing supplements or medicines without professional guidance.
When to seek help
Contact a clinician promptly if the baby is hard to wake, feeding poorly, has markedly fewer wet nappies, appears dehydrated, is repeatedly vomiting, or is not gaining weight. Seek urgent care if the baby is floppy, breathing abnormally or difficult to rouse. Get same-day feeding support when you are worried rather than waiting for the next routine visit.
Questions for your care team
Ask how your baby’s growth and nappies compare with expectations, whether milk transfer can be observed, how to protect supply during supplementation, and when the feeding plan should be reviewed.

Questions people ask
Do soft breasts mean I have low milk supply?
No. Breast softness can be normal once feeding becomes established and does not measure the amount of milk available.
Can frequent feeding be normal?
Yes. Babies may feed often during growth changes or for comfort; growth and milk transfer give more useful information than frequency alone.
