Shortness of breath without wheezing

Breathlessness may come from infection, reduced oxygen exchange, anaemia, an irregular heartbeat, or strain on the heart. Acute bronchitis can cause cough and chest discomfort even when wheezing is absent. Asthma and COPD do not always produce an audible wheeze, particularly between flare-ups. Anaphylaxis may begin with throat tightness, swelling, or sudden breathing difficulty rather than a chest sound. The symptom alone cannot identify its cause.

Stop exertion, sit upright, loosen tight clothing, and notice whether the problem began suddenly or gradually. Record triggers such as walking, lying flat, smoke, fever, or emotional stress, along with chest pain, palpitations, cough, or blue lips. Arrange a clinical assessment if breathlessness is new, recurring, or limiting ordinary activity. A clinician may check breathing, pulse, heart rhythm, oxygen level, and blood tests before deciding whether further tests are needed.

Call emergency services for severe or rapidly worsening breathlessness, inability to speak normally, fainting, confusion, blue or grey lips, sudden chest pressure, or swelling of the face, tongue, or throat. Seek prompt medical care for breathlessness with coughing blood, a fast or irregular heartbeat, high fever, or new one-sided leg swelling. Do not drive yourself if you feel faint or cannot breathe comfortably.
