Understanding exertional breathlessness
Physical activity makes the muscles use more oxygen, so breathing and heart rate rise. Breathlessness becomes more concerning when it appears with an effort you previously managed, arrives sooner than expected, lasts longer after stopping, or is accompanied by other symptoms. Possible contributors include reduced conditioning, asthma, anaemia, respiratory infection, and heart or lung disease. Exertional breathlessness does not identify one cause without an assessment.
Your clinician may ask whether symptoms occur with walking, lifting, sexual activity, or work; whether there is wheeze, cough, chest discomfort, dizziness, palpitations, or swelling; and how recovery feels. Examination and selected blood, heart, or breathing checks can help guide the next step.
Managing the symptom safely
Reduce the pace, take planned rests, and stop the activity if you develop chest discomfort, faintness, or marked dizziness. Avoid smoke and use prescribed inhalers or other medicines only as directed. Record the activity that triggered symptoms, your associated sensations, and recovery time. Arrange a clinical review for a new or persistent pattern, especially if it limits work, walking, or household tasks. Do not test your limits alone when symptoms are unpredictable.
When exertional symptoms are urgent
Call emergency services if exertion causes severe or rapidly worsening breathlessness, chest pressure or pain, fainting, confusion, blue or grey lips, coughing blood, or a very fast or irregular heartbeat. Sudden breathlessness with one-sided leg pain or swelling needs urgent assessment. Seek prompt medical advice if the amount of activity you tolerate is falling, recovery is becoming slower, or symptoms now occur at rest, overnight, or with fever.
