Short answer

Shortness of breath not relieved by an inhaler may be an emergency, especially when breathing is worsening or other warning symptoms are present.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why an inhaler may not help

A reliever inhaler may not help if the flare is severe, the device is empty, the technique is ineffective, or the medicine is not the right treatment for the cause. Breathlessness can also come from infection, anaemia, a heart rhythm problem, angina, a clot, or another condition rather than airway narrowing. Wheeze, cough, chest tightness, fever, palpitations, ankle swelling, or sudden onset provide useful clues, but cannot confirm the cause.

What to do now

Sit upright, stay as calm as possible, and follow the personal action plan prescribed for your inhaler. Check the device and use a spacer if one was prescribed. Do not keep taking extra doses beyond your plan or use someone else’s medicine. If relief does not come promptly, contact urgent medical services and take the inhaler with you.

When to seek urgent care

Seek emergency help for severe or worsening breathing difficulty, inability to speak in full sentences, blue or grey lips, drowsiness, collapse, severe chest pain, or a poor response to your prescribed reliever. Sudden breathlessness, facial swelling, or a widespread rash also requires immediate help.

Questions for your doctor

Ask your doctor to check inhaler technique, explain your action plan, confirm when to repeat a dose, and assess whether infection, heart disease, or another cause is contributing.

Questions people ask

Should I keep using my inhaler if it has not helped?

Follow your prescribed action plan; if breathing remains difficult or worsens, seek urgent help rather than repeatedly adding doses.

Can poor inhaler technique make treatment seem ineffective?

Yes. A clinician or pharmacist can watch your technique and check the device, but severe symptoms need urgent assessment.
