What belongs on a shellfish list

Shellfish are commonly grouped as crustaceans, such as shrimp, crab, and lobster, and molluscs, such as clams, mussels, oysters, scallops, squid, and octopus. A person may react to one group without reacting to every item, so a personal history matters. Cross-contact can occur when seafood shares cooking oil, utensils, grills, or preparation surfaces.

Allergic skin symptoms can include raised itchy welts, generalised itching, flushing, or swelling. A rash after a meal can also come from Contact Dermatitis, Atopic Dermatitis, Drug Rash, Heat Rash, Intertrigo, Psoriasis, Ringworm, or Scabies. Timing, food details, associated symptoms, and examination help separate these possibilities.

What to do after a suspected reaction

Write down the shellfish eaten, the amount, sauces or mixed dishes, the time symptoms began, and any exercise or medicine around the event. Avoid the suspected food until an allergy clinician has reviewed the history, and ask restaurants about shared preparation equipment. Do not challenge yourself at home to test whether the allergy is real.

An allergy clinician may use a focused history, skin or blood testing, and sometimes a supervised food challenge. Testing should be interpreted with the story because a positive result does not by itself prove that eating the food causes symptoms. Ask what emergency plan and medicines you should carry if allergy is confirmed.

When to seek care

Call emergency services immediately for trouble breathing, throat tightness, voice change, faintness, collapse, or swelling of the tongue or throat. Widespread hives with vomiting, severe abdominal pain, wheezing, or dizziness can signal a serious reaction and needs urgent treatment. Do not wait for a rash to worsen before seeking help.
