Sharp pain in uterus during sex

The uterus sits within the pelvis, close to the cervix, ovaries, bladder, bowel, and pelvic-floor muscles. Pressure from penetration can make pain from any of these areas feel uterine. Endometriosis may present with deep pain during sex, while pelvic inflammatory disease can cause pelvic pain with discharge, fever, or bleeding. An ovarian endometrioma may cause one-sided deep discomfort. Dryness from Genitourinary Syndrome of Menopause or pain conditions such as Vulvodynia can add surface pain, even when the main sensation feels deeper.

Stop when the pain becomes sharp. Use positions with shallower penetration, move slowly, and use lubricant if dryness is present. Write down whether pain happens at a particular cycle phase, with deep thrusting, or after sex, and whether it is central or one-sided. Arrange a gynaecology appointment if the symptom repeats, limits intimacy, or comes with bleeding, discharge, urinary symptoms, or bowel symptoms.

Seek urgent care for severe sudden pelvic pain, faintness, heavy bleeding, fever, persistent vomiting, or possible pregnancy. Prompt review is appropriate for new pain after sex with unusual discharge, bleeding, or feeling unwell. A clinician may need to examine the pelvis and consider testing to distinguish infection, ovarian problems, endometriosis, or another cause.

Ask whether the pain is likely to arise from the uterus or a neighbouring structure, and whether examination or ultrasound could clarify this. Mention your menstrual pattern, pregnancy possibility, contraception, prior pelvic infections, and any pain outside sex. Ask which treatment options fit the suspected cause.
