Sharp stomach pain during intercourse: what to know

Pain can occur when a position places pressure on a tender area, when abdominal or pelvic muscles tighten, or when bloating makes movement uncomfortable. Digestive conditions such as Gastritis, Acid Reflux, Gastroenteritis or Food Poisoning may cause tenderness or cramping that intercourse aggravates. Gallstones can cause upper-right abdominal pain, while Diverticular Disease or Crohn's Disease may cause lower abdominal discomfort and bowel changes. These conditions do not explain every episode, and the location, timing, relation to meals, bowel symptoms and any pelvic symptoms help guide assessment.

Stop intercourse when pain begins and rest. Note the exact site, depth, position, intensity, duration and whether pain occurs outside intercourse. Avoid the position that triggers it until you know more, and do not use leftover prescription medicine. Arrange a medical review if pain recurs, lasts after sex, affects daily life, or comes with bloating, vomiting, diarrhoea, constipation, fever, bleeding or pain when passing urine. A clinician can decide whether abdominal, pelvic or digestive assessment is appropriate.

Seek emergency care for severe or rapidly worsening pain, fainting, marked dizziness, a rigid or swollen abdomen, repeated vomiting, heavy bleeding, fever with severe tenderness, or inability to pass stool or urine. Get prompt advice for new pain during pregnancy or possible pregnancy, especially with bleeding or one-sided pain. Do not continue intercourse to test whether the pain returns.

Ask what body area may be causing the pain, whether digestive symptoms change the evaluation, whether an examination or tests are needed, and what symptoms should prompt same-day or emergency care.
