Sharp pain in lower abdomen during sex

Deep penetration can press on the cervix, uterus, ovaries, or pelvic muscles. Endometriosis can present with deep pain during sex, while an ovarian endometrioma can cause pain around an ovary. Pelvic inflammatory disease may cause pelvic pain with unusual discharge, bleeding, fever, or pain when urinating. Genitourinary Syndrome of Menopause and vulvar conditions such as Lichen Sclerosus can make sex painful through dryness, irritation, or fragile tissue. The symptom alone cannot identify its cause.

Stop or change the activity when pain starts. Try positions that allow you to control depth, use adequate water-based lubricant, and avoid fragranced washes or products around the vulva. Note when the pain occurs, its side and location, whether it follows your cycle, and any discharge, bleeding, urinary symptoms, or bowel symptoms. Arrange a gynaecology assessment if pain returns, affects intimacy, or does not settle.

Seek urgent medical care for severe or worsening pain, faintness, shoulder-tip pain, heavy bleeding, fever, repeated vomiting, or a possible pregnancy. Prompt assessment is also sensible for new pain with unusual discharge, bleeding after sex, or pain when urinating. These features can signal a condition that needs examination rather than watchful waiting.

Ask whether the pain seems superficial or deep, whether a pelvic examination or ultrasound would help, and how your cycle or contraception could be relevant. Ask what symptoms should prompt urgent review and whether treatment for the underlying cause could include pelvic-floor therapy, medication, or referral.
