Sex and pain around ovulation: what to know

Ovulation can cause one-sided lower abdominal discomfort as an ovary releases an egg. Deep penetration may make that discomfort more noticeable. The timing alone does not prove that ovulation is the cause.

Pain can also be linked to an ovarian cyst or endometriosis. These conditions can cause recurring pelvic pain, pain with deep sex, painful periods or discomfort that continues beyond the middle of the cycle. A clinician can distinguish these possibilities from your pattern and examination.

Pause sex if it hurts, and try a position with less depth when you feel ready. Record the cycle day, side of pain, duration, bleeding, discharge and bowel or urinary symptoms. A warm compress may soothe mild cramps if it is comfortable for you.

Arrange a routine appointment when pain returns across cycles, interferes with intimacy, or does not settle. Bring your symptom record and mention contraception, pregnancy possibility, prior pelvic infections and any fertility concerns.

Seek urgent care for sudden severe pain, fainting, heavy bleeding, fever, vomiting, a swollen abdomen, or pain with a positive pregnancy test. These symptoms can signal a problem requiring prompt examination rather than simple ovulation discomfort.
