Severe flank pain on both sides: what it can mean

Bilateral flank pain may follow muscular strain, prolonged posture or activity, but it can also occur with kidney or urinary problems. Kidney Infection may produce pain with fever, chills or nausea. Kidney Stones more often cause severe pain on one side, though symptoms can vary. Urinary Tract Infection can cause burning, urgency or frequent urination and may become more concerning when pain moves towards the back or flank. Both-sided pain does not identify one diagnosis by itself.

Contact a clinician promptly when pain is severe, persistent or unexplained. Write down when it started, whether movement changes it, and whether you have fever, vomiting, urine changes, swelling or recent exertion. Drink normally if you are able and have no fluid restriction; do not delay care to try home remedies. Bring a list of medicines and mention kidney problems, pregnancy or recent injury.

Go to urgent care for severe flank pain with fever, chills, confusion, fainting, repeated vomiting, inability to pass urine, blood in the urine, or rapidly worsening weakness. New pain after a significant injury, or pain with severe abdominal symptoms, also warrants prompt evaluation.
