How SCFE presents

Recognising the pattern
The presentation is often subtle at first. A child may limp after activity, shorten the stride, or avoid stairs and running. Pain may be felt in the groin, front of the thigh, or knee rather than directly over the hip. During examination, turning the hip inward may be limited or uncomfortable, and the leg may naturally rotate outward when the hip is flexed. A sudden painful episode can also occur. The presence of these symptoms does not by itself confirm SCFE; other hip, muscle, and joint conditions can look similar. The combination of a limp and reduced hip motion is a reason for timely assessment.
What families can do
Record when the limp began, where the child feels pain, and whether walking is becoming harder. Stop sports and jumping, and avoid asking the child to walk to prove the problem has improved. Arrange medical assessment, with orthopaedic review when SCFE is suspected. Hip imaging helps distinguish a growth-plate slip from other causes. Until reviewed, support the child during transfers and follow advice about pain relief and crutches.
When to seek care
Seek prompt medical care for a new or persistent limp with hip, thigh, or knee pain, particularly if hip rotation is limited. Go to emergency care when the child cannot bear weight, pain rises quickly, or the foot becomes cold, pale, numb, or weak. A red flag is the loss of walking ability, not the assumption that a particular symptom proves a diagnosis.
