Why children may be missed

SCFE occurs while the growth plate near the hip is still open. Children do not always point to the hip: they may mention knee or thigh pain, tire during walking, or gradually stop joining activities. A limp, reduced hip rotation, and an outward-pointing foot are useful clues. The child may protect the painful side by taking shorter steps or leaning the body. Symptoms can affect one hip, while the other hip remains symptom-free but still deserves consideration during assessment. A clinician may ask about growth, recent changes in walking, pain at rest, and any sudden loss of mobility.

The paediatric care plan

Keep the child seated or supported and avoid putting weight through the leg until assessed. Do not let them test the hip with walking, sport, or stretching. A paediatric orthopaedic assessment and imaging establish whether the growth plate has slipped and whether the other side should be checked. Confirmed SCFE is generally stabilised surgically to stop further movement; afterwards, the child receives instructions for mobility, school, wound care, and follow-up. Rehabilitation is introduced according to healing and the surgeon’s restrictions.

When to seek care

Contact a medical service promptly when a child develops a new limp with hip, thigh, or knee pain or cannot move the hip normally. Emergency assessment is needed if walking suddenly becomes impossible, pain becomes severe, or the foot is numb, cold, pale, or weak. These changes can signal a more urgent hip problem and should not be managed by waiting at home.

Ask which movements are restricted, how to get the child to appointments without weight-bearing, and whether both hips were imaged. Discuss the fixation method, expected school accommodations, wound checks, and the milestones used before returning to physical education or sport.
