SCFE clinical presentation: signs to recognise

Short answer

SCFE commonly presents with a limp and pain in the hip, groin, thigh, or knee, often with reduced hip movement and the leg turning outward.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How it may look

The symptoms may build slowly rather than begin with a dramatic event. A child may walk with a limp, avoid stairs, or stop running. Pain can be felt away from the hip, particularly in the thigh or knee, which can make the source easy to miss. Some children report only mild discomfort at first.

On examination, the hip may have less inward rotation. When the hip is bent, the leg may naturally turn outward. Symptoms can affect one side, while the other hip may have no obvious complaint, so both sides usually deserve careful examination.

What to do when signs appear

Arrange prompt assessment for a new limp or unexplained hip, thigh, or knee pain in a growing child. Stop running, jumping, and sport until a clinician has assessed the problem. Note when the limp started, whether pain is constant, and whether the child can bear weight. See [Limping](/symptoms/limping/) for related guidance. If SCFE is confirmed, stabilisation can involve [Fracture Fixation Surgery](/treatments/trauma-and-fracture-care/fracture-fixation-surgery/).

When to seek urgent care

Seek urgent care if the child cannot walk, has sudden intense pain, or becomes unable to move the hip comfortably. Keep weight off the leg while arranging transport. A child does not need to have a fever or visible swelling for a hip growth-plate problem to require prompt attention.

Questions for your doctor

Ask whether the symptoms fit a hip growth-plate problem, which movements should be avoided, whether both hips need imaging, and what changes should trigger immediate review.

Questions people ask

Can SCFE occur without obvious hip pain?

Yes. Referred pain in the thigh or knee and a limp may be more noticeable than hip pain.

Does a limp always mean SCFE?

No. A limp has many possible causes, but a new limp with hip or knee pain needs assessment.
