SCFE causes: why the hip growth plate slips

Short answer
SCFE develops when the upper thigh bone shifts through a weakened hip growth plate, with growth and several health factors influencing vulnerability.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How SCFE starts
The growth plate is an area of developing cartilage between the top of the thigh bone and the developing hip. During a growth spurt, forces across this area can make it vulnerable. The head of the thigh bone stays in the socket while the neck and shaft move out of alignment, creating the appearance of a slip.
Higher body weight can increase the load across the growth plate. Certain hormone or endocrine conditions, kidney disease, or medicines that affect bone development may also be relevant. These associations do not prove a single cause in an individual child, and ordinary activity is not usually something a parent should blame.
What to do about possible causes
Tell the clinician when symptoms began, whether growth or puberty has changed recently, and what medicines or long-term conditions are present. Avoid sports and impact activity until the hip is assessed if there is pain or a limp. A new [Limping](/symptoms/limping/) pattern deserves attention even when there was no fall or obvious injury. Treatment aims to prevent further movement and protect hip function; stabilisation may include [Fracture Fixation Surgery](/treatments/trauma-and-fracture-care/fracture-fixation-surgery/).
When to seek urgent care
Get urgent medical help if pain becomes sudden or severe, the child cannot bear weight, or the leg appears held in an unusual position. Do not test the hip repeatedly or make the child walk. Prompt evaluation matters because continued loading can worsen displacement.
Questions for your doctor
Ask whether an endocrine or kidney assessment is appropriate, whether the other hip needs review, which activities are safe, and how the team will check for ongoing growth-plate problems.

Questions people ask
Is SCFE caused by a sports injury?
It can become noticeable after activity, but the underlying vulnerability is at the growth plate and there may be no specific injury.
Did a parent cause SCFE?
No. Several developmental and health factors can contribute, and assigning blame is not helpful.
