SCFE age range: what parents should know

Short answer
SCFE most often affects children and teenagers during a growth spurt, especially around early to mid-adolescence, but it can occur outside that range; a limp, hip or knee pain, and reduced hip movement need prompt medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical age group
- Growing children and teenagers, commonly around early to mid-adolescence
- Possible first sign
- A new limp, with pain sometimes felt in the knee rather than the hip
- Until assessed
- Avoid running, sports, and unnecessary weight-bearing on the painful leg
What the age range means
Slipped capital femoral epiphysis, or SCFE, is a problem in the growing part of the upper thigh bone where it meets the hip. It is seen most often in older children and adolescents, when the growth plate is still open. A child who is younger or older than the usual age group can still have SCFE, so an unusual age should not be used to dismiss symptoms.
The first sign may be a new limp rather than obvious hip pain. Discomfort can be felt in the groin, thigh, or knee, and the affected leg may turn outward while walking. Symptoms may develop gradually or become severe after a minor incident. The age pattern helps clinicians consider SCFE; examination and hip imaging are needed to assess the growth plate and confirm what is happening.
A child with a limp can have many possible causes. Read more about [Limping](/symptoms/limping/) while arranging an appropriate clinical review.
What to do if SCFE is suspected
Arrange prompt assessment with a clinician experienced in children’s musculoskeletal problems if a child develops a persistent limp, hip or knee pain, or difficulty rotating the leg. Until assessed, reduce walking and running, avoid sports, and do not encourage the child to test the painful leg. Use support during transfers and follow the clinician’s advice about whether the child should remain off the leg.
Treatment depends on whether the slip is stable and how the hip looks on imaging. Many children need an orthopaedic procedure to stabilise the growth plate and prevent further movement. A family may hear about fixation surgery; the related explanation is [Fracture Fixation Surgery](/treatments/trauma-and-fracture-care/fracture-fixation-surgery/), although the exact operation for SCFE is decided by the treating orthopaedic team.
When to seek urgent care
Seek urgent medical care if the child suddenly cannot stand or walk, has rapidly worsening pain, or develops severe restriction of hip movement. Do not make the child walk to reach care. A sudden change can indicate that the hip needs immediate protection and specialist assessment.
Even without severe pain, a new unexplained limp or pain referred to the knee deserves timely review. Early assessment can limit weight-bearing on the affected hip while the diagnosis is clarified.
Questions for your doctor
Ask whether the symptoms and walking pattern suggest SCFE, whether both hips need imaging, and what activity or weight-bearing limits should apply while waiting. You can also ask how the growth plate will be stabilised, what follow-up the other hip may need, and which changes should bring the child back urgently.

Questions people ask
Can SCFE happen outside the usual adolescent age range?
Yes. SCFE is most often identified during adolescence while the growth plate remains open, but age alone cannot exclude it. A limp, hip or knee pain, or reduced hip movement should be assessed regardless of whether the child fits the usual age pattern.
Does knee pain rule out SCFE?
No. SCFE-related pain may be felt in the knee, thigh, groin, or hip. A clinician should examine the hip when knee pain occurs with a limp or altered leg position, particularly during growth.
Should a child with suspected SCFE keep walking?
Avoid unnecessary walking, running, and sport until medical advice is obtained. If the child cannot walk or pain is suddenly severe, seek urgent care and do not force weight-bearing.
