Sausage Toe Osteomyelitis: What to Know

Osteomyelitis is an infection of bone. In a toe, infection can develop after a wound, ulcer, puncture, surgery, or spread from nearby skin and soft tissue. The toe may look diffusely swollen rather than having one small tender spot, and it may feel warm or painful. Redness, drainage, fever, or an open area can occur, but their absence does not rule out bone involvement. A clinician may examine the skin, circulation, sensation, and joints, then use blood tests or imaging to judge the depth of infection.

Arrange a prompt medical assessment, especially if the toe is worsening or you have diabetes, reduced sensation, poor circulation, or a healing problem. Keep any wound clean and covered, avoid squeezing or cutting the area, and use roomy footwear. Do not start leftover antibiotics or stop a prescribed course without advice. Treatment may involve wound care and antibiotics chosen for the suspected organism; deeper infection sometimes needs drainage or removal of unhealthy tissue. Bone grafting may be discussed later when damaged bone needs reconstruction.

Seek urgent care for rapidly spreading redness, severe or escalating pain, fever with chills, pus or a bad smell, black or pale skin, new numbness, or a toe that becomes cold. An open wound over a swollen toe also deserves prompt review. Emergency help is appropriate if you feel faint, confused, or very unwell.

Ask whether the infection appears limited to skin or may involve bone, which tests are needed, how the wound should be dressed, and what changes should trigger urgent review. Ask how treatment will be monitored and when walking or footwear can be progressed.
