Sarcoidosis Skin on the Face: What to Know

Facial cutaneous sarcoidosis may appear as smooth or firm papules, plaques, nodules, swelling around the nose, or changes in colour. It can affect the cheeks, nose, lips, eyelids, or areas around old scars. The appearance may be subtle on some skin tones and may not itch. Acne, rosacea, dermatitis, infection, and other inflammatory conditions can look similar, so a photograph or facial location cannot establish the diagnosis.

Assessment includes the skin pattern and questions about eye irritation, light sensitivity, blurred vision, nasal blockage, breathing symptoms, joint symptoms, and previous sarcoidosis. A dermatologist may recommend a biopsy, and an eye review may be appropriate when symptoms involve the eyes.

Arrange dermatology review for persistent facial bumps, plaques, swelling, or unexplained colour change. Use a mild cleanser, avoid squeezing or harsh exfoliation, and choose simple fragrance-free products. Use sun protection to reduce irritation and contrast from colour changes. Treatment may include a carefully placed steroid injection into the skin for selected local lesions or oral corticosteroid treatment when broader control is needed; the choice depends on examination and overall health. Keep medicines away from the eyes unless specifically prescribed for that area.

Seek urgent care for eye pain, sudden vision change, severe light sensitivity, difficulty breathing, throat or tongue swelling, or rapidly worsening facial swelling. Prompt review is also needed for fever, pus, ulceration, severe pain, or a widespread eruption after a new medicine. Do not wait for a routine appointment if these features develop.
