Possible causes and meaning

Infection may reach the sacrum from a pressure wound, skin or soft-tissue infection, pelvic infection, surgery, injection, or an injury that breaks the skin. Bacteria can also travel through the bloodstream from an infection elsewhere. Diabetes, reduced circulation, a weakened immune system, and prolonged pressure can make infection more difficult to prevent or clear, but the cause cannot be identified from symptoms alone.

Pain in the lower back or buttocks, tenderness, fever, chills, drainage, or feeling unwell may occur. A clinician may use an examination, blood tests, imaging, and sometimes a sample to identify the organism and guide treatment.

What to do next

Arrange prompt medical assessment if sacral infection is suspected. Keep any wound clean and covered as instructed, reduce pressure with regular repositioning, and do not squeeze a draining area. Treatment may involve antibiotics selected for the suspected or confirmed organism, wound care, drainage, or surgery when infected or damaged tissue needs attention. Bone grafting may be discussed later in selected situations where bone reconstruction is required; it is not a substitute for treating active infection.

When to seek care

Seek urgent care for fever with severe sacral pain, confusion, faintness, rapidly spreading redness, pus, or a wound that becomes much deeper or more painful. Immediate assessment is also needed for new leg weakness, numbness around the groin, or loss of bladder or bowel control. People with diabetes or immune suppression should report a new sacral wound early.
