RPR blood test in pregnancy: what to know

RPR stands for rapid plasma reagin. It looks for antibodies that can occur with syphilis. A non-reactive result makes current infection less likely, although your clinician may consider timing and risk. A reactive result does not confirm syphilis by itself: other conditions, past infection or pregnancy-related factors can sometimes affect screening results. A second blood test and your health history help clarify the finding.

Syphilis can pass to a developing baby during pregnancy, so screening is handled promptly. The result should be interpreted with any previous tests, symptoms and treatment history rather than in isolation.

Ask the clinician who ordered the test whether it was reactive or non-reactive and whether a confirmatory test is planned. Do not delay a follow-up appointment while waiting for symptoms, because syphilis may have few noticeable symptoms. If infection is confirmed, treatment can be prescribed during pregnancy and follow-up can include repeat testing and newborn planning. Tell the team about previous syphilis treatment, medicines, allergies and any partner concerns.

Contact your maternity or sexual-health team promptly if the result is reactive, if a partner has syphilis, or if you develop a new genital or mouth sore, an unusual rash or swollen glands. Seek urgent maternity advice for severe abdominal pain, heavy bleeding, reduced fetal movement after you have been advised to monitor it, or feeling acutely unwell. These symptoms do not diagnose syphilis, but they need timely assessment in pregnancy.
