What the pattern can mean

Roseola classically causes a high temperature followed by a fine pink rash as the temperature comes down. It is much more familiar in young children than adults, so a similar adult pattern deserves a careful history and examination. The rash may begin on the chest or back and spread to the neck or arms. It is often mildly itchy or not itchy, and the person may otherwise start to feel better.

A clinician may consider medication reactions, viral illnesses, heat rash, or bacterial skin infection. A rash that is painful, one-sided, blistering, rapidly spreading, or associated with swelling is less typical of a simple roseola-like illness. A photo taken when the rash is most visible can help if it fades before the appointment.

What to do at home

Rest, drink fluids, and keep the skin cool with loose clothing. Use a gentle, fragrance-free moisturiser if the skin feels dry, and avoid introducing new creams until the cause is clearer. Do not scratch; trimmed nails and a cool compress can reduce irritation. Check your temperature and note when the fever ended, when the rash started, where it began, and whether it changes with pressure.

Arrange a GP assessment if the rash persists, returns, or is not clearly improving, especially after a new medicine or recent travel. Until the cause is known, avoid close contact with pregnant people if a contagious viral rash is possible.

When to seek care urgently

Seek urgent medical help for trouble breathing, faintness, confusion, severe headache, a stiff neck, facial swelling, or a rash involving the eyes or mouth. Get prompt assessment for purple or bruise-like spots that do not fade when pressed, rapidly worsening redness, severe pain, pus, or a fever that returns with marked weakness.
