What should you know about risk of recurrent placental abruption?

Short answer

A previous placental abruption warrants an individual monitoring plan in a later pregnancy, but recurrence is not certain.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Past history
Previous placental abruption
What monitoring may include
Blood pressure, placental review, and fetal growth checks
Emergency symptoms
Bleeding, pain, contractions, or reduced movement

Why recurrence is discussed

Placental abruption is separation of the placenta from the uterine wall before birth. It can cause vaginal bleeding, abdominal or back pain, uterine tenderness, frequent contractions, and changes in the baby's movement or heart rate. Bleeding can be concealed, so visible blood is not required for the condition to be serious.

A past abruption gives your clinicians useful information when they plan care, but it cannot predict the course of a future pregnancy. They may consider the timing and severity of the earlier event, blood pressure, smoking, abdominal injury, placental findings, and the baby's growth. Care is adjusted as new information appears.

Planning the next pregnancy

Arrange an early maternity consultation and bring the previous birth or hospital records if available. Explain whether the earlier abruption involved bleeding, pain, high blood pressure, reduced fetal movement, or an urgent birth. Attend all antenatal reviews and follow advice about blood pressure, fetal growth, movement awareness, and medicines. Discuss practical arrangements for reaching the maternity unit quickly. If early birth becomes necessary, the team will consider the baby's gestation, your condition, fetal wellbeing, bleeding, and whether induction or cesarean section is safer.

When to seek care

Go to maternity triage urgently for any bleeding, new abdominal or back pain, a painful firm uterus, regular contractions, faintness, or reduced fetal movement. Call emergency services for heavy bleeding, collapse, severe pain, or breathing difficulty. Symptoms can worsen quickly, so do not wait for a routine appointment or rely on the amount of bleeding to judge urgency.

Questions for your doctor

What details from my previous abruption affect this pregnancy? How often will my blood pressure, placental findings, and baby's growth be checked? Which symptoms require immediate attendance? If early birth is considered, how will the birth method be chosen?

Questions people ask

Can another placental abruption occur without bleeding?

Yes. Concealed bleeding is possible, so pain, contractions, uterine tenderness, or reduced fetal movement need urgent assessment.

Is recurrence inevitable after one abruption?

No. The earlier event guides planning and monitoring, but it does not make recurrence certain.
