What should you know about risk of placental abruption in second pregnancy?

Short answer

A previous placental abruption makes the next pregnancy a reason for closer review, but it does not determine what will happen.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Early separation of the placenta

Understanding the concern

Placental abruption happens when the placenta separates from the wall of the uterus before birth. Separation can reduce oxygen and nutrient transfer to the baby and may cause bleeding, abdominal pain, back pain, uterine tenderness, or frequent contractions. Bleeding may be hidden behind the placenta, so the amount seen does not always reflect the seriousness of the problem.

A previous abruption is part of your pregnancy history and helps clinicians decide how closely to monitor you. Other factors, such as high blood pressure, abdominal injury, smoking, or problems affecting the placenta, may also matter. Your individual plan depends on the earlier event, current symptoms, blood pressure, growth assessments, and placental findings.

How to prepare for pregnancy care

Share the details of the earlier abruption with your obstetric team, including when it occurred, whether bleeding was visible, and how the birth happened. Attend antenatal appointments and follow advice about blood pressure checks, fetal growth, and movement awareness. Take prescribed medicines as directed and discuss smoking, blood pressure, trauma prevention, and any new pain or bleeding. If early delivery becomes necessary, the team may discuss induction of labour or cesarean section according to the clinical situation.

When to seek care

Seek emergency maternity assessment for any vaginal bleeding, sudden abdominal or back pain, a tender or hard-feeling uterus, regular painful contractions, faintness, or reduced fetal movement. Call emergency services for heavy bleeding, collapse, severe pain, or difficulty breathing. Do not drive yourself if you feel weak or dizzy, and do not wait to see whether symptoms settle.

Questions for your doctor

How does my previous abruption change monitoring in this pregnancy? Which symptoms should make me attend immediately? How will fetal growth, placental function, and blood pressure be checked? If delivery needs to happen early, which birth options may be suitable?

Questions people ask

Can placental abruption happen without visible bleeding?

Yes. Blood can remain behind the placenta, so pain, uterine tenderness, contractions, or reduced fetal movement also need urgent assessment.

Will a second pregnancy definitely have another abruption?

No. A prior event informs monitoring and planning, but it cannot predict the outcome of an individual pregnancy.
