Ringworm without itching: what to know

Itching is common with ringworm, but it is not required. A fungal patch may instead be noticed because of its shape, fine scale, colour change, raised border, or gradual outward growth. Other rashes can have the same features, including eczema, psoriasis, irritation, or a pigment change. The absence of itch does not confirm or exclude ringworm. A clinician can examine the border and, when needed, test skin scale before choosing treatment.

Take a clear photo for comparison, note when the patch appeared, and check whether its edge is changing. Keep the skin dry, wash hands after touching it, and avoid sharing towels, clothing, or sports equipment. Do not pick at scale or cover the area with a steroid-only cream without medical advice, as this may make a fungal infection harder to recognise. If ringworm is confirmed, topical antifungal treatment may suit a small area, while oral antifungal treatment may be considered when the infection is extensive, recurrent, or in a location that responds poorly to creams.

Book a clinical review if the patch enlarges, new patches appear, the skin becomes painful or inflamed, or there is no clear improvement with the advised treatment. Seek prompt care for fever, pus, rapidly spreading redness, facial or eye involvement, or a weakened immune system. These features change how urgently the rash should be assessed.

Ask what features support ringworm, whether a skin scraping or another test would help, how long to use treatment, and what change would show that the diagnosis needs reconsideration.
