What should you know about ringworm incognito?

A steroid cream can reduce redness and itching without clearing the fungus. The rash may lose its clear ring shape and become more widespread, uneven, faint, or less scaly. It can return or flare after the cream is stopped. This altered pattern is called ringworm incognito.

The same appearance can occur with eczema, contact dermatitis, or another infection. A clinician may inspect the border and take skin scale for testing, particularly when a rash has changed after steroid use or has not responded as expected.

Tell the clinician exactly which cream was used, where it was applied, and for how long. Do not add a stronger steroid or combine several creams without advice. Management may include a topical antifungal treatment for suitable limited areas or oral antifungal treatment when the rash is extensive, recurrent, or difficult to treat locally.

Keep the area clean and dry, wash hands after applying medicine, and avoid sharing towels or clothing. Use the prescribed treatment for the advised duration, because the surface can look calmer before the fungus has cleared.

Arrange prompt care if the rash is spreading quickly, painful, oozing, near the eyes, on the scalp, or affecting someone with reduced immune defenses. Seek review if it has not improved as expected or repeatedly returns after steroid use. A clinician can check whether infection, irritation, or another condition is driving the change.
