Ringworm in human hair: what to know

Despite its name, ringworm is not caused by a worm. Fungi can infect the scalp and hair shafts, producing an itchy or tender area with flakes, crusting, or short hairs that look broken close to the scalp. Some people develop smooth patches where hair has shed. A scalp infection may look different from a ring-shaped rash on the body, so appearance alone cannot confirm the diagnosis.

The infection can spread through close contact or shared items such as combs, brushes, hats, towels, and pillowcases. Children are often affected, but anyone can develop it. Other causes of scalp scaling or hair loss can look similar, which is why a clinician may examine the scalp and take a sample before choosing treatment.

Arrange a medical assessment if you notice a scaly, itchy scalp with broken hairs or a new patch of hair loss. Scalp ringworm commonly requires an oral antifungal medicine because treatment applied only to the surface may not reach the infected hair shaft. Use medication exactly as prescribed and complete the course unless your prescriber changes it.

Until you are assessed, avoid sharing hair tools, headwear, towels, or bedding. Wash items that touch the scalp, clean combs and brushes as advised, and check household members or close contacts for similar symptoms. Do not scratch or apply steroid cream without guidance, as this can alter the appearance of a fungal infection.

Seek prompt medical care for painful swelling, pus, marked tenderness, fever, or rapidly worsening scalp changes. These features can signal a stronger inflammatory response or a bacterial infection alongside the fungal problem. Contact a clinician soon if hair loss is spreading, the scalp is becoming very inflamed, or symptoms continue despite treatment. Early treatment helps limit transmission and supports recovery of affected hair.

Ask whether the scalp needs testing, which treatment is suitable, how long it should be taken, and whether anyone who shares your home should be checked. Tell the clinician about medicines you already take, liver problems, allergies, pregnancy, or breastfeeding before starting an oral antifungal.
