Why the distinction matters

Ringworm is a dermatophyte infection of the outer skin, hair or nails. It often produces itch, scale, redness and an enlarging border. Bacterial skin infections more often cause increasing warmth, swelling, tenderness, pus, honey-coloured crusting or fever, though appearances can overlap. A bacterial infection does not turn ringworm into a bacterial illness; both problems can be present at the same time. Antibiotics treat bacteria, while antifungal medicines target fungi, so choosing the wrong product may allow the original rash to continue.

Choosing the next step

Keep the area dry, wash hands after touching it, and avoid sharing towels, clothing or razors. For a limited rash that fits ringworm, a clinician or pharmacist may recommend topical antifungal treatment. More extensive disease, infection involving hair-bearing skin, or poor response may require oral antifungal treatment. Do not squeeze, scratch or apply leftover antibiotics, steroid-only creams or mixed products without advice. If there are signs of bacterial involvement, arrange an examination because the treatment plan may need to address both causes.

Warning signs

Seek prompt medical care for rapidly spreading redness, marked warmth, swelling, pus, severe pain, fever, red streaks, or feeling unwell. Arrange a review when the diagnosis is uncertain, the rash affects the face or scalp, or symptoms persist after treatment. Urgent attention is appropriate for swelling near the eye or difficulty breathing.
