Right Ovary Pain During Sex: What to Know

Pain that feels deep on the right during penetration can involve the ovary, fallopian tube, uterus, pelvic lining, bladder, bowel, pelvic floor muscles, or vaginal tissues. Endometriosis or an ovarian endometrioma can cause deep pain, while pelvic inflammatory disease may cause pelvic tenderness, discharge, or fever. A cyst can also create pressure, although not every cyst causes symptoms.

Pain at the opening, burning, or dryness points toward a different group of causes, such as vulvodynia, lichen sclerosus, or genitourinary syndrome of menopause. The timing, depth, position, menstrual pattern, and associated symptoms help separate these possibilities.

Stop or change activity when pain starts rather than pushing through it. Note whether it is at the opening or deep inside, which positions trigger it, and whether it relates to your period. A clinician may offer a pelvic examination, pregnancy test, infection tests, or ultrasound. Lubricant and slower, shallower penetration may reduce friction-related discomfort, but recurrent deep pain still merits review.

Get urgent help for sudden severe pelvic pain, fainting, heavy bleeding, fever, repeated vomiting, or a positive pregnancy test with one-sided pain. Arrange a prompt appointment for new or recurring pain, bleeding after sex, unusual discharge, painful periods, urinary symptoms, or pain that is affecting intimacy. Do not wait for a routine visit if symptoms are rapidly worsening.

Tell your clinician exactly where and when the pain occurs and ask whether the pattern suggests endometriosis, an endometrioma, infection, a cyst, tissue changes, or pelvic floor pain.
