Rheumatoid arthritis of the spine

Short answer
Rheumatoid arthritis of the spine most often concerns inflammation and instability in the upper neck, requiring assessment when neck or nerve symptoms appear.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Area of concern
- The upper neck is the spinal region most associated with rheumatoid arthritis involvement.
- Exercise caution
- Avoid forceful neck manipulation until a clinician has considered spinal stability.
How spinal involvement can present
Rheumatoid arthritis commonly affects peripheral joints, but inflammation can also involve joints and ligaments in the upper cervical spine. Neck pain, reduced neck movement, or pain at the back of the head may occur. Pressure on nearby nerves or the spinal cord is less common but can cause tingling, weakness, clumsiness, balance changes, or altered walking. Lower back pain is not automatically caused by rheumatoid arthritis and may have another explanation.
A clinician may assess neck movement, reflexes, strength, sensation, and balance. Imaging is considered when symptoms or examination findings raise concern. Avoid forceful neck manipulation until spinal stability has been assessed if you have longstanding rheumatoid arthritis or new neurological symptoms.
What to do next
Tell your rheumatology or primary-care clinician about new neck pain, headaches at the base of the skull, arm symptoms, or changes in coordination. Keep taking prescribed treatment unless the prescriber advises otherwise. Choose low-impact activity and keep the neck in a comfortable position; stop an exercise that causes spreading pain, numbness, or weakness. A physiotherapist should know about your diagnosis and any imaging before selecting neck exercises.
When urgent assessment is needed
Seek emergency help for sudden or worsening weakness, difficulty walking, loss of hand coordination, new numbness affecting several areas, or loss of bladder or bowel control. Urgent assessment is also needed after a neck injury when pain is severe or movement is markedly restricted. Fever with a hot swollen joint or feeling acutely unwell needs prompt medical attention because infection can resemble an inflammatory flare.
Questions for your doctor
Could my neck symptoms involve rheumatoid arthritis? Do I need an examination or imaging before exercising? Are there activities or procedures I should avoid? What signs would mean I should seek emergency help? Could my lower back symptoms have a different cause?
