Rheumatoid arthritis in the feet: what to know

Symptoms may affect the toes, forefoot, midfoot, heel area, or ankle. Swelling can make shoes feel tight, while stiffness after rest may make the first steps difficult. Ongoing inflammation can alter pressure across the sole and lead to calluses or tenderness. Foot pain is not specific to rheumatoid arthritis, so a clinician may also consider footwear pressure, tendon problems, injury, nerve irritation, or another joint condition.

Choose shoes with a wide toe box, a stable sole and enough room for swelling; avoid pairs that rub or compress the toes. Check your feet regularly for redness, blisters or breaks in the skin. Gentle ankle and toe movement can maintain comfort, but stop if swelling or sharp pain increases. A podiatry or physiotherapy assessment may help with exercise, insoles, walking changes and joint protection.

Arrange prompt care for a suddenly hot, red, very swollen foot, fever, a new inability to bear weight, or a wound that is not healing. Book a review if pain changes your walking, limits work or sleep, causes repeated falls, or continues despite your usual plan.
