Rhabdomyolysis without dark urine: what to know

Dark or tea-coloured urine can result when muscle pigment reaches the urine, but its absence does not exclude muscle breakdown. Urine appearance is affected by hydration and other factors, and it does not show the full effect on the kidneys or body salts.

Rhabdomyolysis may instead present with muscle pain that is more severe than expected, tenderness, weakness, swelling, stiffness, nausea, or less urine than usual. Diagnosis relies on the history, examination, and blood and urine testing rather than colour alone.

Stop strenuous exercise and contact a healthcare professional promptly if symptoms are unusual or worsening. Explain the activity, injury, heat exposure, illness, medicines, and supplements that came before the symptoms. Drink normally if you are able, unless you have been given fluid restrictions, and do not wait for urine to change colour before seeking advice.

Seek emergency care for severe or worsening pain, marked swelling, new weakness, numbness, inability to move, confusion, fainting, repeated vomiting, breathing difficulty, or very little urine. The absence of dark urine should not delay assessment when these signs are present.
