How they differ
Restless legs syndrome commonly presents with an uncomfortable urge to move, tingling, pulling, or crawling feelings. Resting can bring it on, and walking or stretching may provide temporary relief. A cramp is usually a sudden, painful tightening of a muscle, often in the calf or foot, that can leave soreness afterwards.
Both patterns can have more than one explanation. Iron deficiency anaemia, chronic kidney disease, and peripheral neuropathy may be relevant to restless-leg complaints, while dehydration, muscle strain, pregnancy, or medicines can contribute to cramps. Having cramps alongside restless sensations does not by itself identify a condition.
What may help
During a cramp, gently straighten the leg and draw the toes towards you if this is comfortable; light massage and slow walking may help the muscle release. For restless sensations, try a warm bath, gentle stretching, or a short walk before returning to bed. Keep a note of the exact location, duration, triggers, and whether movement helps.
If episodes recur, affect sleep, or follow a new medicine, arrange a medical review. A clinician may check iron status, kidney function, medicines, and nerve symptoms. Avoid taking iron supplementation unless testing supports it and a clinician recommends it.
When to seek care
Get urgent care for a swollen, hot, red, or very painful leg; sudden weakness or numbness; chest pain; or breathing difficulty. Seek prompt advice if cramps are frequent, last unusually long, cause injury, or occur with persistent weakness. Sleep disruption without these warning signs still merits a routine appointment.
