What should you know about residual DCIS?

DCIS is abnormal cell growth confined to the milk ducts. Residual disease may be found when tissue removed during surgery is examined, or when later imaging or a biopsy identifies an area that still needs assessment. It does not by itself describe an invasive cancer, but it needs careful review because untreated DCIS can sometimes develop into invasive disease.

Your pathology report may describe the area, grade, hormone-receptor findings, and whether the edges of the removed tissue are clear. A breast specialist uses these details with mammogram findings and your previous treatment to explain the choices.

Arrange a review of the pathology report with your breast surgeon or oncology team. The plan may include another breast operation, such as a wider lumpectomy or mastectomy, or radiotherapy after breast-conserving surgery. Hormone therapy may be discussed when the cells have hormone receptors. A biopsy or mammogram can help clarify an area that remains uncertain.

Bring the operative note, pathology report, scan results, and a list of medicines to the appointment. Ask how the recommended option affects breast tissue, recovery, follow-up imaging, and the chance of finding further abnormal cells.

Contact your breast team promptly for increasing redness, warmth, swelling, drainage, wound opening, fever, or worsening pain after a procedure. Seek urgent medical help for severe shortness of breath, chest pain, fainting, or sudden confusion. These symptoms need assessment for complications or another medical problem; they do not establish that DCIS has spread.

Ask: Where is the residual DCIS, and what did the margins show? What grade and hormone-receptor findings are in the report? Would further surgery or radiotherapy improve local control? What follow-up mammogram schedule fits my situation?
