Short answer

Repetitive strain injury in the lower back means pain or stiffness that develops when repeated loading or sustained posture exceeds the back’s current tolerance.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Common loads
Bending, lifting, twisting, reaching, or prolonged sitting
Possible pattern
Local ache or stiffness, sometimes into the buttock
Urgent signs
Bladder or bowel change, saddle numbness, or severe weakness

How this pattern develops

Repeated lifting, bending, twisting, reaching, driving, or sitting can make the lower-back muscles and surrounding tissues sensitive. Pain may feel local or spread into the buttock; stiffness can be worse after staying in one position. Symptoms often vary with workload and movement, and the phrase repetitive strain injury does not identify one exact tissue.

Lower-back symptoms can have several causes, so the pattern, examination, and nerve findings matter. Note whether coughing, walking, lifting, or rest changes the pain, and whether there is leg tingling or weakness.

Ways to reduce the load

Keep gently active within a comfortable range rather than remaining in bed. Break up sitting with short walks, bring loads close to the body, turn with the feet instead of twisting, and use a hip-and-knee bend for lifting. Temporarily reduce the heaviest or most repetitive task, then rebuild it gradually.

Physiotherapy can assess movement, strength, and work demands. Exercise Therapy may help restore trunk and hip control when selected for your symptoms. A Workplace Ergonomic Assessment may be useful if sitting, desk height, vehicle setup, or manual handling repeatedly provokes pain.

When to seek care

Arrange medical assessment if pain is worsening, lasts despite sensible activity changes, repeatedly interrupts sleep, or travels down a leg with numbness or weakness. Seek urgent help for new bladder or bowel control problems, numbness around the inner thighs, severe weakness in both legs, fever with severe back pain, or pain after major trauma.

Questions for your appointment

What movements are safe for me now? Do my symptoms suggest nerve irritation? Which lifting or sitting changes matter most? Would guided exercises help, and what signs mean I should stop or return for review?

Questions people ask

Should lower-back strain mean complete rest?

Complete bed rest is rarely helpful; comfortable movement and reduced aggravating load are generally more useful unless a clinician advises otherwise.

Can desk work contribute to lower-back strain?

Yes. Long, fixed sitting and an unsuitable workstation can increase symptoms, especially when posture is not changed.
