Recurrence of prostate cancer after robotic surgery

After robotic prostate removal, the prostate-specific antigen test is commonly used during follow-up. A rising or detectable result can prompt repeat testing and imaging. Recurrence may be near the operation area, in nearby tissues or in another part of the body. The result alone does not show where cancer is located, so your team considers the pattern over time, the original tumour findings and examination results.

Some people have no symptoms when recurrence is found. Difficulty passing urine, blood in urine, new bone pain or unexplained weakness should be reported, but these symptoms can have causes other than cancer.

Keep scheduled oncology or urology appointments and bring a record of previous PSA results, surgery details and medicines. Your clinician may suggest repeat PSA testing, scans or a biopsy when it would clarify the situation. Options can include radiotherapy, hormone treatment, further surgery or treatment directed at cancer in bone; the choice depends on the location and biology of the recurrence.

Ask whether radiotherapy or brachytherapy is suitable, what side effects may affect urinary or sexual function, and how treatment would fit with your existing recovery. Supportive care can help with pain, fatigue and other symptoms at any stage.

Contact your cancer team promptly for a new or worsening difficulty passing urine, visible blood in urine, persistent back or hip pain, leg weakness, numbness or loss of bladder or bowel control. Seek urgent medical help for inability to pass urine, sudden severe weakness or severe pain. These signs need assessment because they may reflect complications or pressure on nerves, even when recurrence has not been confirmed.

Which PSA pattern or scan result suggests recurrence in my situation? Where might the cancer be, and do I need imaging or a biopsy? What are the aims and side effects of radiotherapy, hormone treatment or another approach? How will urinary, bowel and sexual symptoms be monitored?
