What it may mean

Breathlessness that comes with wheezing, cough, chest tightness, or exposure to a trigger can occur with asthma. COPD, bronchiectasis, acute bronchitis, and other lung problems may cause breathlessness alongside cough or phlegm. Anaemia can reduce the blood’s oxygen-carrying capacity and may cause tiredness, dizziness, or a fast heartbeat. Heart-related causes may bring chest pressure, palpitations, or breathlessness when lying down or walking. Atrial fibrillation can present with breathlessness and an irregular or racing heartbeat. Angina can present with breathlessness, sometimes with chest discomfort. Anaphylaxis can present with sudden breathing difficulty together with swelling, hives, or throat tightness.

The sensation may begin suddenly or build gradually. It can follow exercise, fever, stress, smoke exposure, or a respiratory illness, but new, persistent, or worsening symptoms deserve medical assessment.

What to do next

Stop strenuous activity, sit upright, loosen tight clothing, and move away from smoke or another suspected irritant. Note when the symptom began, what you were doing, whether it is worse lying flat, and whether cough, fever, chest pain, swelling, or palpitations are present. Arrange a clinical review for breathlessness that keeps returning, limits normal activity, or has no clear short-lived explanation. A clinician may listen to your chest, check your heart rate and oxygen level, and decide whether further tests are needed.

When to seek urgent care

Seek emergency help for severe or rapidly worsening breathing difficulty, inability to speak in full sentences, blue or grey lips, fainting, confusion, or severe chest pain. Sudden breathlessness with facial or tongue swelling, widespread hives, or throat tightness may indicate anaphylaxis and needs immediate treatment. Urgent assessment is also appropriate when breathlessness begins suddenly after an injury, occurs with coughing blood, or is accompanied by a very fast or irregular heartbeat. Do not drive yourself if you feel faint or severely unwell.
