Why can ovulation be painful?

Some people feel a one-sided ache or sharp twinge around the middle of the cycle, lasting from a short time to a day or so. The growing follicle can stretch the ovary, and fluid or a small amount of blood released at ovulation may irritate the abdominal lining. The pain may switch sides between cycles or remain on one side.

Pain that is severe, prolonged, worsening, or unrelated to the cycle needs a broader assessment. Endometriosis can cause pelvic pain around periods or ovulation, while an ovarian cyst can cause pressure, sudden pain, or pain with movement. These conditions are possible explanations, not conclusions from the symptom alone.

Track the date, side, duration, severity, bleeding, bowel or bladder symptoms, and whether pain occurs with sex. Rest, gentle heat, fluids, and an over-the-counter pain medicine that is safe for you may ease mild discomfort.

Arrange a clinical review when episodes recur, interfere with daily life, or do not settle. The clinician may examine you and consider pregnancy testing, infection checks, or imaging depending on your symptoms.

Get urgent help for sudden severe pain, fainting, marked dizziness, fever, repeated vomiting, heavy vaginal bleeding, a rigid or very tender abdomen, or pain when pregnancy is possible. These features can indicate bleeding, infection, ectopic pregnancy, or an ovarian problem requiring prompt care.

Ask whether the pattern fits ovulation, whether endometriosis or an ovarian cyst should be considered, which tests are appropriate, and which pain medicines are safe if you are trying to conceive or could be pregnant.
