Rash without fever: what it may mean and what to do

Notice whether the skin is itchy, painful, warm, scaly, raised, blistered, or changing quickly. New soap, detergent, cosmetics, plants, medicines, heat, sweat, or friction can irritate skin. Hives may appear as raised patches that move or fade, while eczema can leave dry, inflamed, itchy areas. A local bacterial infection such as cellulitis tends to cause spreading redness, warmth, swelling, or tenderness and needs medical assessment. Some viral illnesses, including chickenpox, can begin with a rash; fever may be absent, especially early or in milder illness.

Stop any newly introduced skin product and wash gently with lukewarm water. Use an unperfumed moisturiser on dry skin, wear loose clothing, and avoid scratching. Take clear photographs to compare changes and note possible triggers, medicines, foods, or contacts. Do not apply steroid, antibiotic, or antifungal creams to an unexplained rash unless a clinician or pharmacist advises it. Arrange a routine review if the rash persists, keeps returning, or affects sleep.

Seek urgent help for breathing difficulty, swelling of the lips or tongue, faintness, a rapidly spreading rash, severe pain, skin peeling, blisters around the eyes or mouth, or purple spots that do not fade when pressed. Get prompt advice if the rash is hot and tender, follows a new medicine, or occurs in a baby, an older adult, or someone with reduced immunity.

Ask what pattern the rash suggests, whether a medicine or exposure could be responsible, which treatments are suitable for your skin, and what changes should trigger urgent review.
