Rash after fever and vomiting: what to know

Gastroenteritis, food poisoning and rotavirus infection can cause vomiting with fever; a rash may occur alongside an infection or after a new medicine. Meningitis can present with fever, vomiting and a rash, particularly when there is severe headache, neck stiffness, light sensitivity, confusion or marked drowsiness. Migraine may cause vomiting, but a new rash needs separate consideration. Gallstones or pancreatitis are more associated with significant upper abdominal pain than with a rash. The combination cannot be diagnosed from timing alone.

Give frequent small sips of water or an oral rehydration drink if tolerated. Pause solid food briefly if vomiting is active, then offer bland foods in small amounts as appetite returns. Avoid alcohol, greasy meals and unprescribed anti-vomiting medicines. Photograph the rash, note its spread, and list medicines, foods and exposures from the previous day. Wash hands carefully and avoid preparing food for others while vomiting continues.

Get emergency help for a rash that does not fade under pressure, severe headache, stiff neck, confusion, collapse, breathing difficulty, facial swelling, blood in vomit, severe abdominal pain or inability to keep fluids down. Seek prompt care for a child, older adult or pregnant person with repeated vomiting, dry mouth, dizziness or very little urine. Arrange assessment when fever returns, the rash spreads quickly, or vomiting continues despite small sips.

Ask whether dehydration is present, whether the rash needs examination, which fluids and medicines are appropriate, and which symptoms should trigger immediate reassessment.
