What should you know about random knee swelling?

Short answer
Random knee swelling can come from injury, inflammation, fluid build-up, or irritation, and the speed of onset and associated symptoms help determine the next step.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Notice
- Onset, warmth, trigger, locking, and giving way
- Avoid
- Impact, kneeling, deep squats, and twisting during swelling
What the swelling may mean
Swelling soon after a twist may accompany an ACL Tear or Meniscus Tear. A soft fullness behind the knee can occur with a Baker's Cyst. Painful warmth and sudden inflammation can occur with Gout, while pressure over the kneecap may fit Knee Bursitis. Gradual swelling and stiffness may accompany Knee Osteoarthritis. A clinician may need to compare both knees and assess movement, tenderness, stability, and fluid.
What to do during an episode
Reduce impact and avoid kneeling, deep squats, and twisting while the knee is swollen. Elevate the leg and use a wrapped cool pack for short periods if it is comfortable. Keep walking only within a tolerable range; prolonged complete rest can increase stiffness. Note the trigger, onset, warmth, pain location, locking, giving way, and whether swelling changes after activity.
When to seek care
Get urgent care for a hot swollen knee with fever, inability to bear weight, severe pain, a deformed joint, a locked knee, or marked calf swelling and breathlessness. Arrange assessment after a twist, fall, or sports injury, especially when the knee gives way. Recurrent swelling without a clear trigger also merits a clinical review.
Questions for your doctor
Ask what structure may be producing the fluid, whether imaging or fluid testing is appropriate, how much weight-bearing is suitable, and whether Joint Aspiration could help with diagnosis or pressure when fluid is substantial.

No. Swelling has many possible causes, from irritation to inflammatory conditions, but sudden or severe symptoms should be assessed.
Can a swollen knee be exercised?
Choose gentle movement within comfort and pause impact exercise until pain and swelling settle or a clinician advises otherwise.
