Radiological features of psoriatic arthritis

Psoriatic arthritis can affect joints, the spine, fingers and toes, and the places where tendons or ligaments attach to bone. X-rays may show uneven joint-space loss, erosions with adjacent new bone, or a bone-growing response. In the fingers or toes, inflammation can involve an entire digit. Ultrasound can assess active inflammation around a tendon or joint, while MRI can show soft-tissue inflammation and bone-marrow changes before some structural changes are visible.

These findings are interpreted alongside psoriasis, nail changes, pain, swelling and stiffness. Imaging can support the diagnosis and establish a baseline, but a scan by itself cannot determine whether every painful joint is affected by psoriatic arthritis.

Keep a record of which joints swell, how long morning stiffness lasts, skin or nail changes, and whether symptoms fluctuate. Arrange assessment with a clinician familiar with inflammatory joint disease. Treatment may include anti-inflammatory medication, medicines that control the immune process, and physiotherapy to preserve movement, strength and joint protection. Do not stop prescribed treatment solely because an image looks mild; symptoms and inflammation can change between scans.

Seek urgent medical advice for a hot, very swollen joint with fever or feeling unwell, sudden loss of movement, a painful red eye with light sensitivity, or rapidly worsening weakness. These features may need assessment for infection, eye inflammation or another urgent problem rather than being assumed to be a routine flare.

Ask which joints or attachment points look inflamed, whether there is structural damage, what the scan cannot show, and how medication or physiotherapy will be adjusted to your symptoms and examination.
