Pulmonary hypertension and heart failure: what to know

The right side of the heart pumps blood through the lung vessels, while the left side receives oxygen-rich blood and sends it around the body. Disease affecting either side can alter pressure and fluid movement. Breathlessness, reduced stamina, fatigue, cough, palpitations, and swollen ankles may reflect heart or lung circulation problems, but these symptoms do not identify one diagnosis by themselves. Atrial fibrillation, high blood pressure, and valve or lung disease may also influence the picture.

Arrange a chronic disease review if symptoms are changing or everyday activity is becoming harder. Your team may assess weight changes, blood pressure, pulse, oxygen level, kidney function, and signs of fluid retention. Treatment is tailored to the underlying cause and can include medicines, monitoring, rehabilitation, and advice about salt, fluids, activity, and vaccinations. Keep a current list of medicines and tell clinicians about missed doses or troublesome effects.

Call emergency services for severe breathlessness at rest, chest pain, fainting, confusion, blue-grey lips, or rapidly worsening symptoms. Seek prompt medical advice for new swelling, a sudden increase in fatigue, persistent cough, or a sustained racing or irregular heartbeat. Do not wait for a scheduled review if you are struggling to lie flat or are waking because you cannot breathe comfortably.
