Psoriatic arthritis in the foot and ankle

Short answer
Psoriatic arthritis of the foot and ankle may cause swelling, stiffness, heel pain, and difficulty walking.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible symptoms
- Ankle swelling, heel pain, stiffness, or altered walking
- Rehabilitation goals
- Mobility, strength, balance, and comfortable walking
What the pattern can tell you
PsA may inflame the ankle joint, the joints in the foot, or the tissues where tendons meet bone. Pain around the heel or along the sole can make the first steps of the day difficult. Swelling may be visible around the ankle, while stiffness can reduce how far the ankle bends.
Foot and ankle symptoms are not specific to PsA. An injury, tendon irritation, infection, or another joint condition can look similar. Assessment is particularly useful when symptoms affect both sides, recur without a clear injury, or occur alongside psoriasis, nail changes, or symptoms in other joints.
Supporting movement
During a painful spell, reduce high-impact activity and choose steady, low-strain movement that remains comfortable. Wear stable shoes with cushioning and room for swelling. Gentle ankle circles and toe movement may prevent stiffness, but forcing a swollen joint can aggravate symptoms.
A clinician may consider anti-inflammatory medication and a broader PsA treatment plan. Physiotherapy can address ankle mobility, calf and foot strength, balance, and walking technique. A therapist may also advise temporary support or changes to activity while inflammation settles.
When to seek care
Arrange review if foot or ankle symptoms persist, repeatedly interrupt walking, or gradually reduce your activity. Get urgent help for a hot, rapidly swollen joint with fever, severe pain after trauma, inability to stand, sudden calf swelling, or new numbness or colour change in the foot.
Questions for your doctor
Ask whether the pain is coming from the ankle joint, a foot joint, or a tendon attachment. Discuss whether imaging is needed, which activities are suitable now, and how physiotherapy or medication could fit your overall PsA plan.

Questions people ask
Can PsA affect the ankle as well as the foot?
Yes. PsA can involve the ankle joint and nearby foot joints or tendon attachments.
Should I rest completely during a flare?
Usually, protecting the area from aggravating loads while keeping gentle comfortable movement is more useful than prolonged complete rest; seek individual advice.
