Psoriatic arthritis and lung disease: what to know

Breathlessness, a persistent cough, chest tightness, or reduced exercise tolerance should not automatically be attributed to psoriatic arthritis. They may reflect an infection, asthma, heart disease, a smoking-related problem, or inflammation outside the joints. Some treatments also need review when new respiratory symptoms appear. Meanwhile, joint pain, joint swelling, and morning stiffness can limit activity and make normal exertion feel harder even when the lungs are working normally.

A clinician may ask when symptoms began, whether they occur at rest or with activity, and whether fever, wheezing, chest pain, or exposure to illness is present. Examination and, when appropriate, breathing tests or imaging help separate a lung problem from reduced fitness or musculoskeletal discomfort.

Record the pattern of cough or breathlessness, triggers, smoking or vaping exposure, recent infections, and every medicine or supplement you use. Arrange a clinical review if symptoms persist, recur, or interfere with walking or sleep. Do not stop prescribed arthritis treatment without advice. Gentle movement and a tailored physiotherapy plan can preserve joint mobility while the cause of breathing symptoms is assessed.

Bring a list of psoriasis and arthritis treatments to the appointment. A clinician may coordinate rheumatology, respiratory, or primary care input and decide whether medication review or testing is needed.

Seek urgent medical help for severe or rapidly worsening breathlessness, blue or grey lips, fainting, confusion, coughing blood, or chest pain that is severe or unexplained. Prompt assessment is also sensible for new breathing difficulty with fever or marked weakness, especially if you have a weakened immune system.
