Plaque psoriasis may involve the nail matrix, which helps form the nail, or the nail bed beneath it. Matrix involvement can create tiny depressions, ridges, or crumbly growth; nail-bed involvement can cause yellow-brown colour, thickening, or lifting. Affected nails may catch on clothing or become tender with pressure. Fungal infection, injury, and other nail conditions can produce similar signs, so a diagnosis should not rely on appearance alone. Skin plaques or a family history may support the assessment.

Keep nails trimmed without cutting into the surrounding skin, and avoid biting, picking, artificial nails, and aggressive manicures while the area is inflamed. Wear gloves for cleaning and shoes with enough room for toenails.
