Psoriasis on top of the head: what to know

Scalp psoriasis is driven by an overactive immune response that speeds up skin-cell turnover. The result may be a raised area with redness and silvery or whitish scale. It can sit under the hair, reach the forehead or ears, and feel sore when the scale is thick. Scratching can loosen scale but may irritate the skin, cause cracking, or lead to temporary hair shedding from trauma. Dandruff tends to be looser and less sharply defined; a clinician can distinguish these patterns from eczema, fungal infection, or contact irritation.

Use the treatment prescribed for your scalp exactly as directed, allowing it time to contact the affected skin rather than applying it only to the hair. A medicated cleanser, scale-softening product, lotion, foam, or another treatment may be selected according to thickness and extent. Wash gently with lukewarm water, avoid picking, and loosen scale gradually instead of scraping. Fragrance-free products can reduce extra irritation. Track itch, redness, scale, and hair shedding so your clinician can adjust the plan.

Arrange a medical review if the diagnosis is uncertain, the area is spreading, or symptoms persist despite the prescribed plan. Seek prompt care for pus, increasing warmth, marked swelling, fever, severe tenderness, or widespread redness. New joint pain, stiffness, or swollen fingers or toes also deserves mention because psoriasis can involve more than the skin.
