What should you know about psoriasis on the back of the head?

The lower scalp and hairline may develop a clearly bordered patch with thick scale, redness, soreness, or itch. Scale can collect on clothing or bedding, and scratching may break the skin. Dandruff, seborrhoeic dermatitis, eczema, and fungal infection can produce overlapping signs, so location alone does not confirm psoriasis. Look for changes beyond the hairline, nail symptoms, or patches elsewhere and tell the clinician about them.

Use fingertips rather than nails when washing, and loosen scale gently instead of scraping. Follow directions for any scalp lotion, foam, solution, or medicated shampoo; parting the hair helps the product reach the skin. Keep styling products away from sore areas and pause harsh chemical treatments if they sting. Moisturising the hairline may ease dryness, while a clinician can select treatment based on the thickness of the plaque and how much scalp is involved.

Arrange care if the patch is persistent, spreading, very sore, or interfering with sleep, or if scratching causes bleeding. Prompt review is appropriate for pus, crusting, marked warmth, or sudden hair loss in the area. New joint pain or stiffness should also be mentioned because psoriasis can affect joints. Do not assume every scalp scale is psoriasis, particularly after contact with a new hair product.
